I hereby declare that all the information provided in this form is true and correct to the best of my knowledge.

I understand that participation in the Unique Cricket Academy Scholarship Program, including registration, trials, practice sessions, selection matches, tournaments, travel, transportation, and all academy-related activities, is entirely voluntary and at my own risk.

I further declare and agree that:

1. I am physically fit to participate in cricket activities.

2. In case of any injury, accident, illness, disability, medical emergency, or physical harm during trials, practice sessions, matches, tournaments, travelling, transportation, or any academy-related activity, all medical expenses and responsibilities shall be borne by me/my parent or guardian.

3. Unique Cricket Academy, its coaches, staff, management, organizers, sponsors, volunteers, and associated persons shall not be held responsible or liable for any injury, accident, illness, loss, damage, disability, hospitalization, or medical expenses.

4. I am solely responsible for the safety and security of my mobile phone, smartwatch, gadgets, money, cricket kit, valuables, documents, jewellery, and personal belongings.

5. In case of loss, theft, damage, misplacement, or breakage of any personal belongings, Unique Cricket Academy shall not be responsible for any compensation, reimbursement, or replacement.

6. Any expenses arising from injury, treatment, travelling, transportation, accommodation, personal equipment, or any other activity shall be paid by me/my parent or guardian.

7. I agree to maintain discipline and follow all rules, regulations, instructions, and decisions of the academy and coaching staff.

8. The decision of the Selection Committee and Coaches regarding scholarship selection shall be final and binding.

9. I voluntarily participate in the Unique Cricket Academy Scholarship Program and accept all terms and conditions mentioned above without any objection.
